Privacy and Personal Information Protection Notice
Purpose of collection: Council is collecting your personal information in order to determine your eligibility for pensioner concession.
Intended recipients: City of Parramatta Council, Centrelink and Department of Veterans’ Affairs.
Supply: The information contained in this application form and any information requested for the purpose of assessing eligibility for a pensioner concession is required under the Local Government Act 1993 and the Local Government (General) Regulation 2005.
Access/Correction: The information can be accessed by you and may also be available to third parties in accordance with City of Parramatta’s Access to Information Policy and Privacy Management Plan. Your personal information may be disclosed to third parties for the purpose of determining your eligibility for pensioner concession. You may make an application for access or amendment to personal information held by Council. Council will consider any such application in accordance with the Privacy and Personal Information Protection Act.

Declaration and Authority
To verify your eligibility for a rebate, Council will need to confirm your concession card details with Centrelink or the Department of Veterans’ Affairs. Your details may be checked regularly, to confirm ongoing eligibility.
By submitting this form, you:
- give consent to Council checking your concession eligibility with Centrelink or the Department of Veterans’ Affairs; and
- declare that the information provided is true and correct and you are the person making this application. If you make a false statement in an application, you may be guilty of an offence and fined up to $2,220.
